Esophageal cancer is one of the top leading causes of cancer-related deaths in Malaysia. To date, neither the prevalence nor incidence of esophageal cancer nationally have been recorded. Esophageal cancer remains a major and lethal health problem even if it is not common in Malaysia. The late presentation of esophageal cancer makes it a difficult and challenging medical problem. Therefore, more governmental and non-governmental organizations in Malaysia should emphasize primary and secondary prevention strategies (reference: September 2016 – Asian Pacific journal of cancer prevention: APJCP 17(8):3705)
Esophageal cancer was commonest in Chinese ethnicity (41%), followed by Malay (29.9%), Indian (17.4%) and others (11.8%) – ( 2018 data). Esophageal Cancer disease awareness initiatives, campaigns, early detection and multimodality treatment options are essential for improved patient outcomes.
Esophageal cancer is tumour that grows in the food pipe. There are two common types of oesophageal cancer: Squamous and adenocarcinoma, which are two distinct types of cancer that carry different risk factors, biological behaviour, genetic predisposition, treatment and even prognosis.
Fig.1 Oesophagus cancer is cancer that grows in the food pipe.
What causes esophageal cancer?
In general, there are two types of oesophageal cancer. The squamous cell carcinoma and the oesophageal adenocarcinoma. The former comprises 80%-90% of the cases.
Squamous cell cancer is mainly seen in heavy smokers and alcohol users; whereas adenocarcinoma is associated with reflux disease and the precursor of cancer: Barrett’s oesophagus. Other relevant risk factors for adenocarcinoma are obesity, cigarette smoking, family history, male gender, high red meat consumption and low vegetable and fruit intake.
Esophageal cancer is an uncommon cancer. It is ranked 15th and 26th among cancers in male and female respectively. (Based on 2012- 2016 National cancer registry report.)
Where does esophageal cancer spread first?
It usually spreads locally, which means invading deeper through the wall of oesophagus, then it spreads to the regional lymph node and to the distant organ.
Fig. 2 Oesophagus cancer can spread to the deeper layer, then to the lymph nodes and/or the distant organ. (Picture adapted from Oesophageal cancer Tumor-node-metastasis staging, radiology clinic volume 59, issue 2, P219-229, March 01, 2021).
What are the symptoms of esophageal cancer?
Difficulty in swallowing or pain during swallowing and weight loss are the commonest symptoms of oesophageal cancer. In some patients, they have a bleeding tumour, hence leading to vomiting blood, low red cell count or passing blood through the tail end (malena).
Fig 3. Symptoms of oesophageal cancer. (Image taken from oesophageal cancer fund).
Does the disease have a genetic link?
For squamous cell carcinoma, it is rarely linked to genetic disease. On the other hand, around 30% of Barrett’s oesophagus and oesophageal adenocarcinoma are attributable to genetic predisposition.
How is esophageal cancer diagnosed?
Esophageal cancer is usually diagnosed by doing an upper GI endoscopy. This involves a tube that passes through the mouth, throat then the food pipe to examine the stomach and the first segment of the small bowel.
With the advancement of technology, we are now able to detect early cancer of oesophagus better via advanced imaging. This advanced technology improves the resolution and definition of the surface and vessels of the oesophagus lining.
Once the diagnosis of oesphageal cancer is made, the patient will go through a CT scan, PET scan and /or endoscopic ultrasound for the further staging of the disease.

It is important to realize that the treatment option depends on the stage and (biological) aggressiveness of the cancer.
For very early cancer that is localized superficially at the lining of the oesophagus, it can be curative to remove them endoscopically. This requires an interventional endoscopist that is able to delineate the cancer and treat it endoscopically, avoiding major surgery.
Fig 9. An example of endoscopic treatment for an area turning into early cancer. An example of endoscopic treatment for an area turning into early cancer. (Image adapted from https://doctortrindade.com/endoscopic-resection).
When it goes beyond the early stage, the options depend on the type and stage of the disease. In general, surgery, radiotherapy, chemotherapy and a combination of either modality are feasible depending on the condition. In particular, squamous cell carcinoma shows a good response to radiotherapy.
Fig. 11 One of the surgeries that can be done for oesophageal cancer. (Images from
At the very late stage when the disease is advanced, we can endoscopically deploy a stent to open up the obstructed tract, so that the patient can enjoy food during palliative care period.
Fig 12. Endoscopic oesophageal stenting enables the patient to swallow after the procedure. (Image adapted from http://www.drcptanwargastro.com/esophageal-stenting.html).
Is esophageal cancer preventable by avoiding certain risk factors?
Squamous cell carcinoma is predisposed by cigarette smoking and alcohol overuse. Naturally, stopping these risk factors helps in the reduction of the risk. An interesting association between frequent hot beverage drinking with this cancer was found. Hence, frequent use of regular hot drinks such as hot tea and soup is not advisable.
In contrast, adenocarcinoma type is strongly associated with reflux disease. Alongside genetic predisposition, the susceptible patient first develops some cell changes at the lower oesophagus lining, called Barrett’s oesophagus.
Barrett’s oesophagus is a precursor to adenocarcinoma. Hence, if one has reflux disease, the gastroenterology doctor will look for additional risk factors and decide on an endoscopy assessment. The aim is to diagnose Barrett’s oesophagus and to detect early cancer that is amenable to endoscopic treatment at an early stage, avoiding major surgery when the cancer stage is high.
Fig. 13 The process of how reflux oesophagitis develops into Barrett’s oesophagus in a susceptible patient. (Image adapted from https://www.hopkinsmedicine.org/health/conditions-and-diseases/barretts-esophagus).
We also know that obesity and smoking are modestly associated with adenocarcinoma. Again, having a healthy weight, increasing vegetable and fruit consumption, and ending cigarette usage helps to avoid the risk.
What are the survival rates, and influencing factors with esophageal cancer?
In general, the type and stage of cancer, nutritional status and the general health of the patient play vital roles in the prognosis of the patient. Also, the cancer response to treatment such as surgery, chemotherapy and radiotherapy are equally important determinants of the success of treatment.
For example, if the patient has early cancer or even the stage before cancer – high-grade dysplasia, and had clear margin upon endoscopic resection, they have excellent cure and survival rates.
On the other hand, if a patient has aggressive cancer with stage 4 disease with poor health status, then the treatment options become limited, and they have a limited lifespan after diagnosis.

An overwhelming response to the SKSMC Blood Donation Drive shows that Malaysians care for the well-being of the nation.
Klang, Selangor, 8th April 2022 – Sri Kota Specialist Medical Centre Klang in collaboration with HTAR Klang held a blood donation drive at AEON Bukit Tinggi, Klang on Saturday, 2nd April 2022. The drive saw a total of 128 successful donors.
At the event, Pusat Sumber Transplan Nasional joined hands to increase awareness of organ donation to improve the quality of life of people as receiving an organ can become a life-changing incident for a person.
During the full-day event that began at 10 am and ended at 5 pm, donors and families were treated to free basic health screening that included blood pressure checks, blood glucose tests and Body Mass Index checks.
According to the organiser, with hospitals returning to normal and operations being scheduled, hospitals are in need of blood for surgical treatments such as cardiac surgery, organ transplantation, management of thalassemia, haemophilia and patients with anaemia depend on blood transfusion. The efforts to supply enough blood for patients will not succeed without the kind support from the general public for their life-saving gifts of blood.
Various booths were set up to shed health awareness to all visitors.
“Blood donation is a gift of life. We are thankful for the collaboration and support of our esteemed partners, as well the NGOs who championed the event such as Transhub Group of Companies, Persatuan Tan Klang & Pantai Klang, Persatuan Yuan Man, and Gardenia Bakeries (KL) Sdn Bhd. Your thoughtful and caring involvement was truly appreciated”, said the organiser.
During the event, booths were also set up to enlighten the public on organ donation, and various other healthcare issues. Each donor was given a goodie bag as a gesture of appreciation.
The drive saw donors from all walks of life, including those who were medically challenged too.
Sri Kota Specialist Medical Centre is geared to have more blood donation drives with caring sponsors in the future to raise awareness among the community and promote the lifesaving cause. For every blood donation, at least three lives can be saved.

Petaling Jaya, 24 March 2022 – KL Wellness City Sdn Bhd has unveiled its first-of-its-kind integrated mixed-development township focusing on sustainable healthcare. The project, known as KL Wellness City, is set to be developed and completed over a 10-year period in Bukit Jalil. The township has a gross development value (GDV) of more than RM11 billion and spans 26.49 acres across seven plots of land.
KL Wellness City spans 26.49 acres over seven plots of land with an overall gross development value (GDV) of more than RM11 billion.
KL Wellness City, a sustainable township supported by a full suite of offerings across the health, medical, fitness, and wellness pillars, was introduced by Managing Director Dato’ Dr Colin Lee. It will transform the way Malaysian and international patients receive care. Medical care is linked with general healthcare, preventative care, and fitness throughout KL Wellness City in order to enhance each resident’s and patient’s potential for health, wellness, and fitness.
The integrated township has been thoughtfully designed to allow all residents to live a healthy and balanced lifestyle by integrating the built environment with nature, green areas, convenience, safety, and the opportunity to nurture good and healthy habits. The building design maximises natural light and ventilation with extensive commercial and retail infrastructure and services, as well as an abundance of green vegetation. Meanwhile, the township offers a high occupant-to-nature ratio, with up to 5m2 of greenery per person, embracing nature’s calming and therapeutic qualities.
“We are pleased to introduce the first Healthcare and Wellness City in Southeast Asia, right here in Kuala Lumpur. Through its 360-degree ecosystem, we aim for KL Wellness City to offer an impeccable experience for everyone seeking a more efficient and pleasant patient journey. This can be achieved by integrating the medical, ancillary and support services into one purpose-built wellness hub like the one we are offering. We also aim for this healthcare hub to further strengthen Malaysia’s position as the leading destination for global healthcare tourists, and to contribute towards Malaysia’s economy,” said Dato’ Dr Colin Lee, Managing Director, KL Wellness City Sdn Bhd.
Integrating Healthcare Delivery
KL Wellness City intends to improve the patient experience for Malaysian and international patients by improving the way medical professionals operate and manage their clinics. The Nobel Healthcare Park, a dedicated medical and wellness suite complex with a GDV of RM1 billion, is being built as a one-stop destination from diagnosis to rehabilitation to achieve this. The Nobel Healthcare Park, which is directly connected to the International Tertiary Hospital via a two-level link bridge, connecting medical suites and ancillary medical support services such as diagnostic, therapeutic, and custodial services to improve the efficiency and performance of the entire healthcare delivery ecosystem.
KL Wellness City deviates from the traditional hospital-centric healthcare delivery model by promoting holistic wellness and preventative healthcare throughout the patient journey as well as for medical personnel. This philosophy is reflected in KL Wellness City’s nature-centric architecture, which includes a huge stretch of greenery, landscaped rooftop breakout areas with restaurants, cafes, an auditorium, a swimming pool, natural open-air lift lobbies, and wheelchair-accessible covered walkways.
Specialists practising in The Nobel Healthcare Park will have full access to the services of the International Tertiary Hospital, which is expected to open in early 2025 and will include 624 beds scalable to 1,000, cutting-edge equipment and operating theatres, as well as innovation labs and clinical R&D facilities. The integrated IT infrastructure system provides medical practitioners with a full range of complementary and ancillary medical services, as well as faster results and reporting.
The Nobel Healthcare Park’s medical suites are built to Ministry of Health regulations and are structured in a modular format, allowing experts to choose the perfect size for their practice, with suites starting at 223 sq ft and prices starting at RM414,000. Owners have complete control over their practices, including determining professional fees, administering medications, and hiring people, as well as utilising the medical equipment at the International Tertiary Hospital to save on expenses. The suites in The Nobel Healthcare Park also offer a unique investment opportunity, enabling pioneering doctors who invest in purchasing medical suites to benefit from the projected strong capital appreciation.
“The medical suites in The Nobel Healthcare Park have been designed based on the core management team’s own experience in the medical field. We have infused the design with elements that offer practising medical specialists the flexibility to optimise the use of space to suit their precise requirements and the freedom to make changes according to evolving needs. We are confident this will enhance the specialists’ interest in owning the suites as an investment in addition to running a practice here. In fact, we are expecting a take-up rate of 80%, within the first three months of the first phase’s official launch,” elaborated Dato’ Dr Lee.
The Nobel Healthcare Park also has fully-furnished wellness suites, which are versatile living spaces that can be used for owner occupancy, home office use, or short-term stays, such as step-down care and healthcare traveller accommodation, with top-tier leisure and lifestyle amenities like a 50-meter lap pool, jacuzzi, sandy cove, and F&B outlets. These suites, which come in 268 sq ft and 386 sq ft options, starting from RM338,000, offer additional opportunities for attractive rental yield, for medical professionals offering the space to patients and their families, or for other real estate investors.
Future-centric technology and standards
The latest technology will be used at KL Wellness City to improve efficiency and service delivery, emulating the Smart City concept, in which the smart use of healthcare technology, networking, and real-time data improves patient and resident convenience. Meanwhile, the township’s forward-thinking attitude is reflected in its environmental, social, and economic sustainability credentials, which are reinforced by careful planning and management to address social, environmental, and economic implications. Built to the exact specifications of GBI certification guidelines, KL Wellness City’s long term green credentials are further supported by Building Information Modelling (BIM 6D) up to the highest level of asset management, to enjoy a high level of operational efficiency during construction and long-term operation.
Complementing Malaysia’s economic goals
KL Wellness City’s concept aligns with Malaysia’s position as the leading global destination of choice for healthcare tourists, as well as the goals of the Malaysia Healthcare Travel Industry Blueprint 2021-2025, which aim to elevate medical tourism in the country. It is led by a strong management team with decades of experience in building and managing tertiary hospitals. Malaysia’s medical tourism sector produced RM1.7 billion in revenue from 1.22 million healthcare travellers in 2019, and the industry has the potential to contribute up to RM7 billion to the economy by 2025, aiming to achieve economic recovery beyond pre-pandemic levels.
“Malaysia’s medical care is among the best in Southeast Asia, and our International Hospital is benchmarked against top hospitals around the region, particularly Mount Elizabeth Novena in Singapore and Bumrungrad International Hospital in Bangkok, both of which serve a clientele comprising significant numbers of international patients. We are confident that our thorough planning for this fully integrated and interconnected township will contribute towards a positive experience for all who seek health and wellness at KL Wellness City,” Dato’ Dr Lee added.
