    Role of Hospital Medical Director

Dr. T. Selvi Moorthy, Medical Director, Sri Kota Specialist Medical Centre, Klang.

A Medical Director (MD) is primarily a clinician who is in charge of the standards of medical care given to patients in a hospital. The MD oversees the compliance of regulations in a healthcare institution, supervises its day-to-day operations and provides leadership to ensure it runs efficiently and smoothly.

An interview with Dr. T. Selvi Moorthy, Medical Director of Sri Kota Specialist Medical Centre, Klang

Is the medical director of a hospital a practising doctor?

Yes, in most instances the Medical Director sees patients clinically and undertakes administrative functions. There are some instances where Medical Directors do not directly see patients clinically but, in such positions, the Medical Director is indirectly involved in patient care when making decisions.

The medical director still ensures that he /she keeps abreast with the current medical practices and has their practising license renewed every year.

What are the differences between a medical director and a medical doctor?

A Medical Director structures and coordinates the administrative areas to ensure the performance of doctors, nurses, pharmacists and facility operations are all in line to assure quality and efficient clinical services are provided for optimal patient care, needs and safety.

While a Medical Doctor is a licensed medical practitioner who sees patients clinically, the doctor will examine patients, diagnose illnesses, monitor patient progress and administer a treatment plan related to the patient’s health and well-being.

What are the responsibilities of a medical director?

In a hospital environment, the medical director heads the medical staff organization and is responsible for the effective functioning of all clinical services. The medical director is usually the legal representative of the hospital and the Person in Charge responsible for compliance with the rules, and ethical and statutory requirements laid out by the Ministry of Health.

The medical director ensures that doctors and nurses are all credentialled and privileged to work in the designated areas with strict adherence to policies for good nursing and supportive care to all patients. The job function also involves overseeing ethical and medicolegal issues if any, and strictly ensuring proper upkeep and confidentiality of patient records and documents.

The medical director is responsible for effective communication within the hospital, mitigating concerns to elevate the quality of patient care and promote Continuing Medical Education for all clinicians and medical staff to improve professional competence for high care standards.

Together with the Hospital Administrator [CEO/Hospital Director], the medical director liaises with government and non-governmental bodies in licensing, and ensuring national health policies placed are adhered to.

Does the medical director work closely with clinicians and staff for improved patient care?

Yes, Medical Directors in hospitals work closely with clinicians, management and staff to ensure clinical quality is maintained, patient safety and care being of utmost importance in daily affairs.

Clinicians and staff are very much involved when making policies and procedures relevant to day-to-day operations. Regular audits are held, incidents reported and the root cause analysis completed and shared to minimize errors in future.

Do medical directors manage the patient crisis, and make informed decisions?

Effective leadership is essential in any crisis. It is primarily a shared responsibility of the Hospital Administrators [CEO/Hospital Directors] to manage a patient crisis, subject to the gravity of the issue.

But for general patient concerns or common repetitive problems arising from a day-to-day operation, the hospital has in place standing orders, rules and regulations to address the concerns.

Risk management teams work hand in hand with the Medical Director and Hospital Director/CEO to manage all kinds of crisis situations from experience and anticipated risks.

In certain emergency situations, if either one is convinced that the risks involved are proportionately more than the benefits, then shared decision-making is implemented by having immediate meetings and discussions with all concerned parties.

Does the medical director function as a facilitator within the hospital organization to achieve a patient and family-centred environment?

Yes, of course. Facilitating communication and ensuring proper decision-making is key to a hospital organization’s success. The Medical Director ensures the care of a patient is not compromised by having systems in place for patient grievances. The medical director sometimes steps in to act as a facilitator, or even a mediator by scheduling meetings with the patient’s doctor and family should the need arise.

With the management, the Medical Director ensures that problems are addressed as quickly as possible by facilitating creative and effective communication among interdepartmental working teams.

Both the Medical Director and Hospital Administrators are eager to work with managerial colleagues as equal partners to maintain clinical quality, ensure patient safety and improve the patient experience.

Breast Cancer: Early Detection and Screening

Dr Ahmad Muhsin Mohammad Nor, Consultant General Surgeon – Sri Kota Specialist Medical Centre, Klang.

Did you know that breast cancer afflicts an estimated figure of more than ONE MILLION women in the world each year and is the 2nd leading cause of cancer deaths among women worldwide, second to lung cancer? Breast Cancer is the most common form of cancer affecting women in Malaysia. About 1 in 19 women in this country are at risk, compared to 1 in 8 in Europe and United States. Early detection and getting appropriate medical attention can help save lives (ref: https://www.breastcancerfoundation.org.my/about-breast-cancer)

The Malays have the lowest incidence of 33.9 per 100,000. This translates into 1 in 16 Chinese, 1 in 16 Indian and 1 in 28 Malay women developing breast cancer at some stage in their lives (ref: https://pubmed.ncbi.nlm.nih.gov).

Breast Cancer is the most common form of cancer affecting women in Malaysia.

An interview with Dr Ahmad Muhsin Mohammad Nor, Consultant General Surgeon – Sri Kota Specialist Medical Centre, Klang.

      What is breast cancer?

Breast cancer is a type of cancer that originated from the breast. The cancer cells, when left untreated, will continue to grow and can spread to other parts of the body such as liver, lungs, bones and to the brain. It is one of the commonest causes of cancer death in Malaysia.

What are the symptoms of breast cancer?

Patients with breast cancer may exhibit symptoms such as a breast lump with or without breast pain. Some patients may notice some blood-stained discharge coming out from the nipple. The nipple may also turn inwards. Others have symptoms such as skin dimpling, or skin changes at the breast and lumps felt at the underarm. Patients with advanced breast cancer may have bone pain such as at the spine, a distended abdomen due to accumulation of abdominal fluid and yellowish discoloration of the eyes and skin.

What are the risk factors for breast cancer?

A risk factor is anything that raises your chance of getting a disease. There are many risk factors for breast cancer. Some risk factors cannot be controlled while others can be changed. A person who has a family member with breast cancer such as her sibling or her mother is at risk of developing breast cancer as well. Age is a risk factor. An older woman is more at risk than a younger aged woman. Other risk factors include women who reached puberty early as well as menopause at a much later age than normal. Women who don’t have children are at a higher risk as well as those who have children but do not breastfeed them. Another risk factor is those who are on oral contraceptive pills or hormone replacement therapy for a prolonged period. Other factors that increase your risk are related to lifestyles such as having regular alcohol intake and being overweight after menopause.

      How is breast cancer diagnosed?

There are 3 things that doctors need to do in general to diagnose a breast cancer. This is called triple assessment. The first is to do a thorough physical examination of both breasts and the armpits to look for breast lumps and lumps in the armpit. Then, the patient will need to undergo an imaging either a mammogram or an ultrasound or both to further visualize the breast lumps and look at the characters of the lumps via the imaging. And finally, a sample of the lump will be taken (biopsy) and sent to the laboratory for definitive diagnosis. This is achieved by pricking the breast lump with a small needle to take the sample. A local anaesthesia may be given to alleviate the pain before sampling is done.

Can early detection and screening prevent breast cancer?

Yes. Screening means to do tests or examinations on people who have no symptoms. Early detection means that the disease is detected earlier, most usually before the symptoms are felt. It is recommended that women do regular breast self-examination at least once a month in front of the mirror to check for any breast lump. Do seek an appointment with the doctor whenever there is a breast lump or changes to the breast noted during the examination. It is also recommended to do a screening mammogram once you reach 40 years of age to screen for breast cancer. A mammogram is a form of x-ray which is done at the breasts to detect any abnormalities. Early detection and removal of the lump can prevent breast cancer.

A mammogram is a form of x-ray which is done at the breasts to detect any abnormalities.

How can I lower my chances of getting breast cancer?

Some risk factors for breast cancer are not modifiable such as age, family history of breast cancer, age of puberty and menopause, and the number of children you have. However, certain factors like prolonged use of oral contraceptive pills or hormone replacement therapy need to be discussed further with your doctor especially if you already have other unmodifiable risk factors. Keeping a healthy lifestyle is important including regular exercise to maintain a healthy body weight and avoidance of alcohol.

      Can menstruation affect breast cancer?

Menstruation itself does not cause breast cancer. However, studies have shown that the higher number of menstrual cycles a woman has over time will increase the risk of breast cancer. Hence, the earlier a person reaches puberty, the higher the risk. Women who have no children will have more menstrual cycles and will be at a higher risk. Also, those who had not reach menopause at a late age of their life or those taking hormone replacement therapy are at a higher risk due to a prolonged exposure to the hormone oestrogen and progesterone.

      Will breast cancer affect fertility?

Breast cancer itself does not affect fertility. However, during the treatment of breast cancer, a patient may need further therapy such as radiotherapy, chemotherapy and hormonal therapy. These therapies are not to be given in a woman who is pregnant, thus those with breast cancer may need to postpone their plan to bear a child during those duration of treatment. Certain types of chemotherapy are known to cause infertility as well. Hence, it is crucial for those who plan to get pregnant after breast surgery to discuss further with their doctor.

      How is breast cancer treated?

The mainstay of treatment for breast cancer is surgery to remove the cancer. This can either be by removing part of the breast that is affected or removal of one side of the breast altogether. This depends on several factors such as the location and size of the breast lump, as well as the willingness to undergo radiotherapy if only part of the breast is removed. An imaging such as a computed tomography (CT) scan will further show if the breast cancer has metastasized elsewhere such as the liver, lung, bone or brain. Based on the stage and types of the cancer, some patients may need chemotherapy, radiotherapy with or without hormonal therapy after the surgery.

How often should I do a breast-self exam?

It is recommended for all women to do breast self-examination at least once a month to check for breast lump. This is preferably done in front of the mirror to look for any asymmetry of the breast or any skin changes that is previously not present or not seen before. Do seek further treatment by your doctor if there are any breast lumps felt or if there are any changes to the breast seen.

Everbridge marks three years of supporting Australia’s largest wildlife rescue organisation

Everbridge, Inc. (NASDAQ: EVBG), the global leader in critical event management (CEM) and national public warning software solutions, today announced the company is marking three years of proud support for Australia’s largest wildlife rescue organisation, WIRES (Wildlife Information Rescue and Education Service), a non-profit providing rescue and rehabilitation for sick, injured, and orphaned native animals for over 35 years. More than 5,200 WIRES volunteers – across four states in Australia and comprised of ten different wildlife rescue organisations – use Everbridge and its xMatters digital service availability solution to communicate and coordinate wildlife rescues.

Subscribe to our Telegram channel to get a daily dose of business and lifestyle news from NHA – News Hub Asia!

“The recent severe flooding across Queensland and New South Wales was unprecedented in scale and impact on communities, the environment, and our native wildlife,” said Lucy Clark, Emergency Response Manager for WIRES. “In March alone, WIRES received around 10,000 rescue calls. During this period, Everbridge proved to be invaluable yet again, providing us with the ability to get notifications out to volunteers in affected areas in a timely and efficient manner. We had hundreds of volunteers and key staff in these areas, many of whom were carrying out search and rescue. We were able to quickly create new communications groups to target for rescue notifications, allowing us to reach our primary response teams, maintain regular contact with them, and allocate urgent resources to the flood zones.”

Continued Clark, “Access for rescue teams was extremely difficult in the immediate aftermath, and we needed to focus on 24/7 phone support for people dealing with displaced wildlife in addition to urgent logistical support for our volunteers. Many animals in care needed to be evacuated. Without Everbridge working away in the background, this task would have been impossible. Hundreds of exhausted animals were rescued, including orphaned joeys, wombats and bandicoots, echidnas, and many waterlogged birds. We believe that with our coordinated emergency response efforts and our sophisticated Everbridge software, we provided the best possible outcomes for our precious wildlife. We are so grateful for your support and state-of-the-art technology. Together we can achieve extraordinary things to ensure more animals get the second chance at life in the wild that they deserve.”

WIRES, in partnership with Everbridge, continues to expand its reach. Through this “collaborative rescue” program, volunteers from other animal rescue organisations are incorporated into the Everbridge-WIRES platform, allowing WIRES to instantly broadcast notifications to appropriately trained volunteers, no matter what organisation they are from, getting help to more animals even faster.

“Donating our services to WIRES was an easy decision for us,” said Steve Foster, Everbridge Vice President – Australia, New Zealand & Oceania. “It’s critical that the growing number of WIRES volunteers receive and share the incident information they need to save these animals, many of which are already on Australia’s endangered species list. It is a no-brainer to help enable organisations like WIRES to supplement their skill sets in the same way we assist IT professionals in a business environment.”

In addition to supporting WIRES animal rescue and rehabilitation efforts, Everbridge powers the emergency alert system for the entire country of Australia, providing population-wide alerting to inform and protect the continent’s 34 million residents and annual visitors. Globally, more local, state, and national governments deploy the Everbridge Public Safety solutions than any other provider, offering the ability to reach over two billion residents and visitors in more than 200 countries and territories.

UTM vice-chancellor’ message to the world: Work in-silo no more, upgrade skills, take action

The Vice-Chancellor of Universiti Teknologi Malaysia (UTM), Professor Datuk Ts Dr Ahmad Fauzi Ismail was invited by the Government of Australia to speak at the World Energy Forum in Sydney, Australia, held from 12th July to 14th July 2022. He was speaking as one of the panel speakers for the session on ‘Workforce and Skills’.

The Australian government extended the invitation to Professor Fauzi because of his outstanding achievement in bringing UTM to a greater height, especially in leading UTM in producing quality graduates for the Energy sector by making the university curriculum responsive to the needs of the energy-related industries.

                    Work Closely with Industries

When speaking to the forum participants, he said that Malaysia had led the production of the number of energy managers required by a nation.

In responding to the question by the forum moderator, on the reasons for Malaysia’s outstanding performance on the highest number of renewable energy workers in Indo-Pacific countries, he responded that close collaboration with the industries is the key to achieving it.

In UTM, for example, students can work with the industries through the platform of the Consortium of Academia, Industry, Government and Society for Synergistic Transformation or the acronym AIMS4STAR. UTM has created this platform for all relevant parties, especially industries, to make the university’s future graduates ready to work in actual settings of the future workplace.

UTM students are exposed to the industries’ work culture and learn to understand the actual work in industries. UTM strongly believes universities should collaborate closely with relevant parties to provide necessary training to ensure that future graduates are skillfully prepared for the appropriate workforce.

Upskilling and Reskilling, APEL and Micro-credential

The UTM Vice-Chancellor also highlighted the need for graduates’ upskilling and reskilling programmes to meet current and future workforce requirements. Some graduates will need to upgrade their skills through the upskilling and reskilling process to ensure they are marketable and capable of doing actual work in today’s industrial scenario.

In Malaysia, the platform of Accreditation of Prior Experiential Learning or APEL was introduced by the Malaysian government to recognise the previous experience gained by an individual to enrol as a university student.

APEL will likely be able to change the higher education landscape that traditionally relied on formal academic qualifications, such as having a Bachelor’s degree to pursue a Master’s degree. Through APEL, an individual may be able to enrol as a Master’s degree student without having a Bachelor’s degree by recognising his or her working experience.

Besides, the Micro-credential (MC) platform can be used to transfer credits from MCs taken by an individual to shorten his or her study period. MC is gaining popularity because of the concept of “chunking” a particular massive course to smaller ones (piece-by-piece) using several modules, which is likely to be favoured by those individuals who are too busy with work.

                    “Walk the Talk”

Speaking to the audience during the forum, the UTM Vice-Chancellor asserted that thoughts and preaches should be translated into action. He is popular for his strong statement of “Walk the Talk” that he strongly believes in and practices. As a university vice-chancellor, he does not want the university lecturers or staff to merely ‘talk’ without acting upon it. His messages delivered in the forum session were crucial to ensure the sustainability of higher education, especially for the Energy sector.

What he conveyed to the forum audience can be summarised as follows: Work In-Silo No More, Upgrade Skills, and Take Action. These three critical points should be remembered by all parties, especially the community of academics, to ensure the relevance of higher education institutions for other parties, especially industries, governments, and societies.
